From a private home
Coordinate with hospice or the appropriate authorities, then arrange a respectful transfer after release.
Care wherever your family needs to begin
We coordinate pickup from private homes, hospitals, hospice settings, nursing facilities, and other locations, including transfers with local providers anywhere in the United States.
Thoughtfully coordinated
Your loved one does not need to be at a hospital for us to help. Call Horizon Funeral Home day or night with the location and circumstances. We explain the next steps, confirm the required authorization and release, and coordinate transfer into care. For locations outside our local service area, we work through qualified local funeral providers.

Service options
Coordinate with hospice or the appropriate authorities, then arrange a respectful transfer after release.
Work with hospital, nursing facility, hospice, or medical examiner staff on release and pickup instructions.
Arrange local care and the onward transfer with qualified providers and a confirmed receiving funeral home.
Understanding the service
For an expected death at home under hospice care, follow the hospice team's instructions and contact us so the release and transfer can be coordinated. For an unexpected death, or if you are unsure whether the person has died or needs urgent medical help, call 911 first. Required pronouncement, official review, and release must occur before a funeral transfer can proceed.
Tell us the exact location, whether the person is at a residence or facility, the contact responsible for release, and the name of the person authorized to arrange care. At a home, practical details such as stairs, elevator access, building instructions, or a difficult approach help the transfer team prepare respectfully.
We coordinate local transfers in the New York area and arrange with qualified local providers when care begins elsewhere in the United States. The professionals, timing, authorizations, distance, and charges depend on the location and circumstances. Calling at any hour does not promise an immediate arrival or a fixed response time.
Once care and release are arranged, we can discuss a funeral in Brooklyn, cremation, transportation to another state, or repatriation abroad. When another funeral home already has your loved one in its care, we coordinate with that provider and the authorized family so the transfer instructions and receiving arrangements are clear.
Exact authorizations, timing, provider requirements, and outside charges are confirmed for each arrangement by the funeral director.
What to expect
Explain where your loved one is and whether hospice, a facility, or an official authority is involved.
The director coordinates authorization, required release, the appropriate transfer provider, and practical access details.
We coordinate care and then guide the family's funeral, cremation, or onward transportation plan.
